• Transmission risk without intervention: 15-45% (breastfeeding increases risk).
• With optimal intervention (ART + ARV prophylaxis + formula feeding): <1% transmission.
• Maternal ART: All HIV+ pregnant women should receive triple ART regardless of CD4 count (start as early as possible, continue lifelong).
• Infant ARV prophylaxis: Nevirapine or AZT daily from birth for 4-6 weeks (or longer if breastfeeding).
• Infant testing: HIV DNA PCR at birth, 6 weeks, and 6 months (or as per national guidelines).
• Breastfeeding: If safe, affordable formula is available → avoid breastfeeding (zero risk). If not (resource-limited settings) → exclusive breastfeeding for 6 months with continued maternal ART and infant ARV prophylaxis.
• Mode of delivery: Elective C-section at 38 weeks if viral load >1000 copies/mL at 36 weeks. If viral load <1000, vaginal delivery is safe.
• Cotrimoxazole prophylaxis: For HIV-exposed infants from 6 weeks until HIV infection excluded.
