Transmitted by tsetse fly (Glossina). Stage 1 (hemolymphatic): fever, lymphadenopathy (Winterbottom sign), chancre. Stage 2 (meningoencephalitic): sleep disturbance (daytime somnolence, night insomnia), neurologic symptoms. Treatment depends on stage and subspecies: stage 1 β pentamidine (gambiense) or suramin (rhodesiense); stage 2 β NECT (nifurtimox-eflornithine) for gambiense, melarsoprol for rhodesiense. Without treatment, fatal.
